Monthly Fee - $125

One time enrollment Fee - $75

                                                                     All the following services are included:

90 minute evaluations for diagnosis and treatment planning. Includes recommendations for medications as well as referrals to therapy, other medical specialists and community resources.   

30-60 minute follow up appointments - you decide. We will discuss symptoms, medication benefits, side effects and adjustments as needed. Supportive counseling is provided. 

Completing Short Term Disability, educational accommodation letters, Emotional Support Animal letters and Doctor excuses for work and school, prior authorization for medications, releasing medical records. 

Ordering bloodwork and evaluating results. 

Genetic testing for medications is available. 

Medication interventions to assist in recovery from addiction such as Suboxone and Vivitrol. This provider can provide education and treatment with these and more.
